People who are borked out of their mind on drugs can have a vacant look. Some antipsychotic medications can cause a person to have a vacant look. Any mental illness that causes a person to lose touch with reality can cause a vacant look, like schizophrenia. And some people can have a vacant look in their eyes simply because they are wrapped up in their own thoughts and not paying attention to what’s going on around them.

Most of the people who work at rehabs have had substance abuse issues of the own. You won’t see many psychologists working at rehabs, you will find psychiatrists because they can write prescriptions, psychologists can’t.
